Chirurgické riešenie extrémnych varixov dolných končatín

Autori opisujú prípad extrémnych varixov dolných končatín s ťažkou chronickou venóznou insuficienciou (CHVI) v štádiu C5 – C6 (podľa CEAP klasifikácie) po morbídnej obezite u 32-ročného pacienta. Varixy boli hrúbky 10 – 12 mm a cirkulárne postihovali obidve DK, brušnú a hrudnú stenu. Trofické zmeny, hyperpigmentácia a lipodermatoskleróza boli prítomné na dolných troch štvrtinách oboch predkolení. Pacient bol už ako 22-ročný plne invalidizovaný kvôli morbídnej obezite (225 kg). V priebehu troch rokov schudol na 85 kg. U pacienta bolo nutné riešiť nielen extrémne varixy dolných končatín s ťažkou CHVI, ale aj následky enormného schudnutia, ktoré sa prejavilo nadbytkom kožných lalokov na bruchu a hrudníku, ktoré vyžadovali aj plastické korekcie. V priebehu jeden a pol roka sa vykonalo u pacienta sedem operácií.

Surgical treatment of extreme varicose veins in lower extremities

The authors describe the extreme case of varicose veins of lower extremities with severe chronic venous insufficiency (CVI) in stage C5 – C6 (according to CEAP classification) after morbid obesity in 32-year old patient. Varices were 10 – 12 mm thick. Both lower extremities were circularly affected as well as abdominal and chest wall. Trophic changes, hyperpigmentation and lipodermatosclerosis were presented in the lower three quarters of both legs. Patient was disabled already as 22-year old fully for morbid obesity (225 kg). During three years he reduced his weight to 85 kg. The patient was treated not only for extremely varices of lower extremities with severe CVI, but also for the enormous consequences of losing weight, which led to an excess of skin flaps on the chest and abdomen, which required plastic correction. During one and a half year, seven operations in patient were performed.
