Rozsáhlá retroperitoneální lymfadenopatie s průvodní nefralgií jako projev raritně se vyskytujícího „burned-out“ germinálního tumoru varlete

Kazuistika prezentuje případ pozdní diagnostiky raritního, takzvaného burned-out germinálního tumoru levého varlete. Rovněž poukazuje na problematiku těchto zřídka se vyskytujících tumorů, kdy k diagnostice dochází až v pokročilém stadiu a příznaky onemocnění jsou již způsobené metastazováním. „Vyhořelý“ germinální tumor je definován jako přítomnost metastáz germinálního testikulárního tumoru s histologickou regresí primárního testikulárního ložiska. Důležitým prvkem v léčbě těchto pacientů je odstranění postiženého varlete z důvodu vysokého procenta perzistence nádorových buněk i po chemoterapii.

Extensive retroperitoneal lymphadenopathy with accompanying renal pain as a manifestation of rare „burned-out” germ cell testicular tumor

This report documents the rare case of burned-out germ cell tumor of the left testicle. It also highlights the problem of this rare tumors, when the correct diagnosis is made at the advanced stage of the disease, and the symptoms are often caused by the metastatic process. Burned-out germ cell tumor is defined as the presence of metastasis of germ cell tumor with histological regression of the primary testicular tumor. An important part of the treatment of this patients is removal of the affected testicle because of high percentage of persistence of cancer cells, even after chemotherapy.
