MANAGEMENT OF OBESE PATIENTS WITH CARDIOMETABOLIC RISK
Management of obese patient with cardiometabolic risk must be oriented toward decreasing of intraabdominal fat together with treatment of associated traditional risk factors of cardiovascular disease. Therapy of obesity is oriented to lifestyle modification, which is the cornerstone of the treatment of obese patients. Recommendations to obese patient should include optimisation of the meal plan and enhancement of physical activity. Psychological approach may be helpful. If this approach is unsuccessful, pharmacology therapy may be considered (rimonabant, sibutramin, orlistat). For morbid obesity there is possible role of bariatric surgery. It is also necessary to treat diabetes mellitus (the best drug seems to be metformin), to treat hyperlipoproteinemia, arterial hypertension and there is a place also for antiplatelet therapy.
