Current practices in the treatment of acute ischemic stroke in the 21st century

In recent years, the options for acute treatment of ischemic stroke have expanded significantly. In addition to the commonly available acute treatment (intravenous thrombolysis), endovascular therapy has been added, which continues to develop. In addition to mechanical thrombectomy, acute treatment of severe stenoses and dissections of the afferent cerebral arteries is also commonly performed. The time-frame and selection of patients considered for acute treatment is constantly expanding with the introduction of advanced imaging techniques. However, in the environment of dramatic technical progress, there has been a long-term discussion about the options for implementing acute treatment as soon as possible and as effectively as possible. In this paper, we mention some original studies, their clinical relevance, currently valid indication criteria and research trends. We also briefly present areas within acute management that can be performed more efficient or where time delays can be minimized.
