Perioperative complications in thoracic surgery patients with metabolic syndrome
The Metabolic syndrome is characterized as a cluster of metabolic and cardiovascular risk factors: diabetes and insuline resistance, abdominal obesity, dyslipidaemia and high blood pressure. The perioperative risk of each of the components of metabolic syndrome are well known. This study is focused on complex problematics of a metabolic syndrome in the perioperative period of thoracic surgery. Patients included in the study were randomised from the group of the thoracic surgical inpatients undergoing elective surgery. The intraoperative incidence of cardiopulmonary complications was not statistically significant. The incidences of postoperative hypoxaemia (p<0.002), postoperative hypercapnia (p<0.001), mechanical ventilation (p<0.031) and the postoperative incidence of cardiovascular complications were significantly (p<0.031) higher in the group of patients with metabolic syndrome. The incidence of hyperglycaemia >10mmol/l was also significantly higher (p<0.001). The incidence of complications was independent as far as gender, type and duration of the surgery and comorbidity are concerned. The epidural analgesia did`t have influence on the incidence of cardiovascular, respiratory and metabolic complications.
