Abdominal sepsis and septic shock – surgical view
The treatment of sepsis is constantly being developed and modified. Abdominal source of sepsis is the second most common and, considering the clinical presentation and specifics, can represent a diagnostic and therapeutic challenge with the need for interdisciplinary management. The article provides a brief summary of the diagnosis of sepsis, recommendations for drug treatment according to the guidelines for the treatment of sepsis from 2021, and surgical planning for procedures in the abdominal source of sepsis. The basis of surgical treatment remains debridement, removal of infected foreign bodies, drainage of infected fluid collections, decompression of the abdominal cavity and resection of the affected organ with reconstruction of the continuity of the gastrointestinal tract. Relaparotomy on demand or temporary closure of the abdominal cavity with a vacuum system with fascia traction appears to be the best surgical treatment strategy.
