Misplacement of chronic haemodialysis catheter in common carotid artery

Introduction:Chronic haemodialysis catheters are one option of vascular access for patients in end-stage renal disease. They can be inserted percutaneously or surgically. Aim. Authors describe a rare complication which occurred after percutaneous insertion. The catheter was placed in common carotid artery. The complication was treated by operation in general anaesthesia. The catheter was extracted and the lesion in common carotid artery was reconstructed with venous patch and a new catheter was placed into internal jugular vein. Conclusion. Percutaneous placement of chronic haemodialysis catheters is preferred nowadays because of lesser invasivity. But the chronic catheters for haemodialysis are thicker than usual central venous catheters and the results of misinsertion of chronic catheters can be more serious. Therefore catheters for chronic dialysis should by inserted into deep vein by ultrasound guidance. The other possibility is open surgical insertion of catheter, especially by patients after repeated multiple insertions of chronic haemodialysis catheters.
