Iliofemoral deep vein thrombosis – the most important cause of pulmonary thromboembolism: therapeutic guidelines

Iliofemoral deep venous thrombosis (DVT) represents a major cause of morbidity and as a part of venous thromboembolism also of mortality in current population. The current therapeutic strategy contents mainly the anticoagulation therapy in combination with elastic compressive therapy. However, they are not sufficient enough to prevent the post-thrombotic syndrome (PTS). International guidelines stress the role of endovascular techniques in the management of extensive DVT. In properly selected patients the endovascular techniques are effective in early thrombus removal, and hence they have better ability to prevent development of PTS, as well as to maintain the better long-term patency of venous system. Guidelines are useful and important tool for the diagnostic and therapeutic strategy, however, the key role still remains the physician’s decision in individual clinical situation.
