Radical antegrade modular pancreatosplenectomy versus conventional distal pancreatosplenectomy in patients with tumors located in the left part of the pancreas: a retrospective analysis of a single-center cohort

Introduction: Radical antegrade modular pancreatosplenectomy (RAMPS) is considered an effective procedure for left-sided pancreatic ductal adenocarcinoma (PDAC). Since RAMPS was first published by Strasberg in 2003, it has attracted more and more attention in this type of disease. However, the limited number of suitable cases for this modality makes it difficult to perform any prospective randomized study compared with the standard procedure. Material and methods: In this article, the authors present their experience with this procedure and retrospectively compare the data with conventional distal pancreatectomy (DP). A total of 27 patients are included, of which RAMPS was performed in 12 cases. Results: Analysis of our cohort revealed that RAMPS correlated with a higher number of lymph nodes harvested, without significantly increasing operative time or milder complications. Conclusion: RAMPS is a safe method for the treatment of left-sided pancreatic cancer, in which a higher R0 resection can be achieved and a higher number of lymph nodes removed.
