Intraventricular meningiomas – a retrospective analysis of a rare disease

Introduction: Intraventricular meningiomas are a rare type of tumor, accounting for 0.5% to 3% of all intracranial meningiomas. In 80 percent of cases, they most often originate from the trigone of the lateral ventricle, which is close to eloquent cortical areas. Methodology: At the Neurosurgical Clinic of UNLP and the Faculty of Medicine of UPJŠ, we operated on 1869 brain tumors in the years 2010–2022. Of this total number, 392 were meningiomas, with intraventricular meningiomas in 5 patients (1.27%). Of the 5 patients, 4 were women. The average age of the patients at the time of surgery was 46 years. Retrospective data from the course of hospitalization, surgical records, imaging examinations, and histology from samples of the operated tumors were analyzed. Results: In our five patients, total and complete resection, verified by postoperative magnetic resonance imaging, was possible in all five patients. The risk of complications was very low, 2 patients in the postoperative period had epiparoxysms, well controlled by antiepileptic treatment. Conclusion: Surgical removal of intraventricular meningiomas represents a significant challenge in modern neurosurgery due to their proximity to eloquent structures, especially the optic tract. The gold standard in the treatment of symptomatic intraventricular meningiomas remains microsurgical resection, most often from the parietooccipital approach. In case of recurrence, stereotactic radiosurgery is possible.
