Recommended case management of patient by addictological services – case report
In the presented case story, we want to point out the process of working with a demanding client, who is characterized by a proactive relationship of the case manager (doctor, psychologist, social worker) to the patient. The work with the presented methods in the case study is an opportunity for unification resp. linking health and social care and also for a better understanding of the patient‘s lifestyle.
