Maintenance intravenous solutions in children

Current pathophysiological perspective considers euvolemic hyponatremia to be caused by excess of pure water which cannot be eliminated by the organism. In clinical practice, the administration of hypotonic solutions to children has become a routine standard. In hospitalized children, especially in patients after surgery, infusion of hypotonic solutions is (due to inappropriately increased secretion of antidiuretic hormone; ADH) associated with a high risk of development of iatrogenic hyponatremia. This has led to a fundamental change in the recommendations for maintenance intravenous solutions in children, which currently prefer administration of isotonic fluids.
