Radiotherapy of pelvic lymph node in high risk prostate cancer and local treatment in metastatic disease

Pragmatic clinical overview of local and regional prostate cancer radiotherapy, which may be helpful in decision making in equivocal situations is provided in the article. Five scenarios are related to regional pelvic lymph node irradiation in high risk prostate cancer, two post prostatectomy, two in primary radiotherapy and one in oligorecurrent nodal involvement. Other two conditions include prostate irradiation in metastatic disease and metastasis directed therapy in oligorecurrent cancer. Available medical evidence is integrated with theoretical assumptions arguing for or against regional and local treatments. Clinical practice in respective cases at high volume reference center is summarized.
