Initial treatment of metastatic renal cell carcinoma
In general, systemic therapy is indicated when unresectable disease, either metastatic or locally advanced, is present. For patients with good-risk disease, antiangiogenic targeted therapy is the primary option. Among the available agents, the preferred ones are sunitinib and pazopanib. Both drugs have similar efficacy in comparison in randomised clinical trials. According to the guidelines from the European Association of Urology (EAU), which will be updated based on the recent data, for patients with intermediate or poor-risk disease immunotherapy with the combination of nivolumab plus ipilimumab is preferred where this regimen is available. If immunotherapy is not available, antiangiogenic targeted therapy remains the primary option.
