Double-hit diffuse large B-cell lymphoma
The tumour is a disease of the cellular genom which determinates its biological behaviour. The diffuse large B-cell lymphomas (DLBCLs) with a concurrent rearrangements of MYC and BCL2 or BCL6 genes (double-hit DLBCLs) or all three genes (triple-hit DLBCLs) have a very aggressive clinical course and unfavourable prognosis despite an intensive immunochemotherapy. DLBCLs with a high protein expression ofMYC and BCL2 (double-expressor DLBCLs) without rearrangements of their genes have the similar biological features too. Their identification is possible by the concurrent examination of the bioptic specimens by immunohistochemistry and by cytogenetic techniques. Till the optimal mode of management these patients is not determined.
