Inhalative vasodilatators in treatment ARDS Nitrogen monoxide (iNO) – case report

Inhalative nitrogen monoxide (iNO) is the only clinically useful and effective vasodilator pulmonary vascular bed that influence selectively lung circulation and has important part in treatment of ARDS. Our department used iNO as a part of comprehensive treatment of ARDS on a patient after bariatric surgery that led to pneumonia and ARDS. Invasive measurement of hemodynamics was indicating symptoms of severe pulmonary hypertension, that was refractory to medication and ventilation treatment. Due to these results, inhalation iNO was used with positioning the patient on the abdomen. After 9 days of ventilation and using iNO patient was successfully extubated.
